# Invoice input sheet

Fill this in before you ask any tool to generate an invoice. Everything below
is an input you own. Nothing here should be guessed for you.

Invoice requirements differ by country. This sheet is a checklist for gathering
your own facts, not tax or legal advice. Confirm the mandatory fields, numbering
rules, registration numbers, and any electronic-invoicing obligation with your
own tax authority or an accountant in your jurisdiction.

## 1. Who is issuing

- Legal name (exactly as registered, if you are registered):
- Trading name, if different:
- Address where legal documents can be served:
- Email and phone:
- Tax or VAT registration number, if you have one:
- Company registration number, if you have one:

## 2. Who is being billed

- Customer legal name:
- Customer billing address:
- Customer contact name and email for accounts payable:
- Customer tax or VAT number, if your rules require it:
- Purchase order or contract reference the customer needs to see:

## 3. Invoice identity and dates

- Invoice number (and the numbering scheme it comes from):
- Invoice date (date of issue):
- Supply date, or the period the work covers:
- Payment due date, or the terms that produce it (for example, 30 days):

## 4. Line items

One row per billable thing. Repeat as needed.

## 5. Tax treatment

- Are you required to charge tax on this supply? (yes / no / not sure)
- Rate or rates that apply, and to which lines:
- Any exemption, zero rate, or reverse-charge wording your rules require:
- Where you got that answer (tax authority page, accountant, tax software):

If you cannot fill in this section from a source you trust, stop and get the
answer before you send the invoice.

## 6. Money already moved

- Deposit or advance already paid:
- Credits, discounts, or retainers to deduct:
- Amount actually due after those deductions:

## 7. Payment instructions

- Bank details, payment link, or accepted methods:
- Late-payment terms or interest, if your contract sets them:
- Remittance reference the customer should quote:

## 8. Checks that must pass

Ask for these as formulas in the workbook, not as typed numbers.

- Sum of line totals equals the subtotal.
- Tax equals the rate multiplied by the taxable base.
- Total equals subtotal plus tax.
- Balance due equals total minus deposits and credits.
